Device
Operating System
Browser
Wallet

This also helps alot:

Press F12 to open dev tools in your browser and select "Console"
Refresh the page and try to recreate the issue/problem you had
Take screenshot including the Console or any other helpful info
Gather any other clues you find and include them in your post

This Bug Reporting section of the Den, is the best place to report bugs. Trying to provide support through Telegram alone is very difficult, especially as the chat can be very busy.
